OFFICIAL RULES
NO PURCHASE NECESSARY ELIGIBILITY: This Sweepstakes ("Sweepstakes") is open to United States residents who register at www.skinclinical.com/myreversestoryterms andconditions
Void where prohibited by law. The Drawing is governed by United States law, and is subject to all applicable federal, state and local laws and regulations. Employees of SkinClinical. ("Sponsor"), its affiliates, subsidiaries, advertising, promotion and fulfillment agencies and members of the immediate family or household of each are not eligible.
PRIZES/ODDS: One (1) winner will receive a prize of a trip to Beverly Hills, CA - inclusive of airfare, 2 night stay in hotel and 1 day of spa services up to a thousand dollars of credit at Thibiant Beverly Hills Spa. ALL EXPENSES, TAXES AND COSTS ASSOCIATED WITH ACCEPTANCE AND USE OF ANY PRIZE NOT SET FORTH IN THESE RULES ARE THE OBLIGATION OF THE WINNER.
Odds of winning depend on the number of eligible entries received. No assignment, transfer or substitution of prize is permitted.
ENTRY: Entry begins Oct 7, 2016 and continues through March 15, 2017 9:30 pm EST. ("Sweepstakes Submission Period"). During the Sweepstakes Submission Period you can enter at www.skinclinical.com/myreversestory Users must provide their name, phone number, and email address to enter. The Sweepstakes is open only to those who fully complete the entry form, whose names will be entered into the drawing pool.
WINNER SELECTION: Winner will be selected in a random drawing from all eligible entries received. Drawing will be conducted on or about Feb 1, 2017; May 1 2017; by a representative of the Sponsor, whose decisions shall be final and binding on all matters relating to the Sweepstakes. Odds of winning depend on the number of eligible entries received.
VERIFICATION OF WINNER: Winner must continue to comply with all terms and conditions of these Official Rules. Winning is contingent upon fulfilling all requirements. The winner will be notified in-person at the date of the judging. The winner will be required to sign on-site to Sponsor, an affidavit of eligibility, and liability/publicity release in order to claim his/her prize. If the winner cannot be contacted, fails to sign and return the affidavit of eligibility and liability/publicity release within the required time period or if the prize or prize notification is returned as undeliverable, winner forfeits prize. In the event that a winner of a Sweepstakes prize is disqualified for any reason, Sponsor may award the applicable prize to an alternate winner. The Sponsor will repeat this process until the prize is awarded. A person is not a confirmed as the Sweepstakes winner until they complete all steps outlined in this paragraph.
ALL POTENTIAL SWEEPSTAKES WINNERS ARE SUBJECT TO VERIFICATION BY SPONSOR WHOSE DECISIONS ARE FINAL AND BINDING IN ALL MATTERS RELATED TO THE SWEEPSTAKES.
RIGHTS GRANTED BY YOU: By entering this content you understand that SkinClinical, anyone acting on behalf of SkinClinical, or its respective licensees, successors and assigns will have the right, where permitted by law, without any further notice, review or consent to print, publish, broadcast, distribute, and use, worldwide in any media now known or hereafter in perpetuity and throughout the World, your entry, including, without limitation, the entry and winner's name, portrait, picture, voice, likeness, image or statements about the Sweepstakes, and biographical information as news, publicity or information and for trade, advertising, public relations and promotional purposes without any further compensation.
GENERAL: All taxes on prizes and all expenses related to acceptance and use of prizes not specified are the sole responsibility of winners. By participating, entrants agree: (a) to these rules and decisions of Sponsor, which shall be final in all respects; and (b) to release, discharge and hold harmless Sponsor from any and all injuries, liability, losses and damages of any kind resulting from their participating or their acceptance, use or misuse of a prize including, without limitation, personal injury, death and property damage, and claims based on publicity rights
copyright, trademark, defamation or invasion of privacy. Sponsor is not responsible for and all not be liable for: (a) electronic, hardware or software program, network, Internet or computer malfunctions, failures, or difficulties of any kind, including without limitation, server malfunction or by any human error which may occur in the processing of entries, or the incorrect or inaccurate capture of entry or other information, or the failure to capture, or loss of, any such information; (b) failed, incomplete, garbled or delayed computer transmissions; (c) late, lost, misdirected or incomplete entries; or (d) any condition caused by events beyond the control of the Sponsor that may cause the promotion to be disrupted or corrupted. Sponsor reserves the right in its sole discretion to cancel, terminate, modify or suspend the Sweepstakes or any portion thereof should computer hardware or software malfunctions (such as but not limited to virus, bugs, worms, tampering, unauthorized intervention, fraud, technical failures) or any other causes corrupt the administration, security or proper conduct of the Sweepstakes, and select the winners from entries received prior to the action taken or in such other manner as deemed fair and appropriate by Sponsor.
ADDITIONAL CONDITIONS: Participation must be human initiated. Automated and/or repetitive electronic submission of views (including but not limited to those made using any script, macro, or bot) will be disqualified and transmissions from any related e-mail or IP addresses may be blocked. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the Sweepstakes; to be acting in violation of the Official Rules; to be acting in violation of the terms of the Web site, or to be acting in a non- sportsman like or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person. In case of dispute, the authorized subscriber of e-mail account used to enter the promotion will be deemed to be participant and must comply with these Official Rules. Authorized account subscriber is deemed to be natural person who is assigned an email address by an Internet Access Provider, online service provider, or other organization that is responsible for assigning email addresses.
CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE PROMOTION MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.

DISPUTES: Participant agrees that: (a) any and all disputes, claims and causes of action arising out of or in connection with the Sweepstakes or any prizes awarded shall be resolved individually, without resort to any form of class action, and participant shall consent to the exclusive jurisdiction of the state or federal courts of San Diego County California; and (b) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering any Sweepstakes, but in no event attorneys' fees.
